Purchase Agreement -

(Sales contract) with all signatures. Should include a legal description from the survey, deed, or title work.

EMPLOYMENT

Employer Info -

Name of company, address, and personnel director for all employers for last two years.

Pay Stubs -

Most recent month's stubs. Must be typed or computer-generated and include your name, Social Security number, year-to-date earnings, and employer's name.

W-2 Forms -

From all employers for last two years.

Signed Federal Tax Returns -

If self-employed or paid on commission, bonus, or interest income (last two years).

OTHER INCOME

Alimony, child support, or separate maintenance income need not be revealed if you do not wish to have it considered as a basis for repaying your loan.

Pension, Disability, and Social Security -

Current letters from source stating amount received and last two years' 1099s.

Support -

Copy of divorce decree or separation agreement, property settlement, and proof of child support.

Rental Income -

Copies of leases and last two years' tax returns.

ASSETS

Bank Statements -

Last two months' statements from each bank where you have an account.

Other Assets -

Full descriptions of all other assets (i.e. IRA's, 401k's, profit sharing).

LIABILITIES

List of all creditors -

Addresses, account numbers, monthly payments, and outstanding balances.

HISTORY

Landlord Address -

Addresses along with rent receipts or cancelled checks for the past two years.

Current Mortgage Payments -

Cancelled checks for the last twelve months; or last year-end statement plus cancelled checks for elapsed months since statement was issued.

Bankruptcy (if applicable) -

Legible copies with status and explanation.

Explanation Letter (if applicable) -

Letter explaining any late payments, judgments, liens, bankruptcy, or foreclosures.

FHA/VA

Picture ID with Social Security number.

VA only

DD-214, certificate of eligibility, or statement from your Commanding Officer if you are on active duty.
